PAJC is here to help with any required paperwork, including school forms, pre‑participation sports forms, and any forms for camps and activities. If you’re transferring care or need office documents completed, our team is happy to assist.
Preparticipation forms required for extracurricular activities may be completed at your child’s annual wellness visit or within the same calendar year once the health history form is submitted. Please allow 24–72 hours for processing.
If you are a new patient, we require previous medical records. Please complete this form to request records from your prior pediatrician, or you may send the completed form to us and we will forward it on your behalf.
If you are transferring care, please complete this form and return it to us so we can forward your records to your new physician.
